Attached is the questionnaire for the Dominican divorce process. Please
complete each section fully and click the Submit Form Button when complete.
If a particular section does not apply, such as children, property, debts,
etc. simply skip that section. Please complete all the sections that are bolded.
You must send
us the following documents when you sign the papers:
Original
Certified Copy of the Marriage License;
Original
Certified Copies of Birth Certificates for any minor children.
Copies of your driver's license or what you used to notarize the papers with the notary stamp and signature.
The following documents will be required when you travel to the Dominican
Republic:
A passport.
American
citizens need to obtain a visitor's Visa at the airport from which you depart.
Unless already
paid please mail a check or money order or provide credit card information
for our deposit of $1,000.00. The remaining $950.00 (or $100 more per child) can be paid after
you set a hearing date. Our office will prepare the papers. Administrative
charges of a minimum of $200 apply if you or this office terminates our
services prior to typing your paperwork. Please remember that we have
a series of minimum charges that apply ($600 or more depending on the
status of your work) should you later cancel this proceeding. Our fee
does not cover travel costs, room, food and other miscellaneous expenses
during your short stay in the Dominican Republic, or other unrelated services
you may request from the Dominican attorneys. Once your papers have been typed our typing fees have been fully earned.
